Home Health Plus will help individuals regain their strength and make them as independent as possible after recovery from an illness.

• Skilled Nursing – Our registered and licensed vocational nurses coordinate care services to monitor patients' progress, including pain management and health teachings.
• Home Health Aides – Our certified home health aides assist patients with activities of daily living.
• Physical Therapy – Our physical therapists manage and provide instructional programs to increase patients' ambulation and mobility.
• Occupational Therapy – Our occupational therapists evaluate and provide ways to increase muscle strength to aid patients with their activities of daily living, allowing them to be as independent as possible.
• Speech and Language Pathologist – Our licensed speech pathologists assess a patient’s ability to communicate and provide instructions to regain communication skills. Techniques of swallowing will also be provided.
• Medical Social Work – Our certified medical social workers make the necessary coordination to make use of the available community resources.
